Report Identity Theft
Has your identity been stolen, or do you believe your personal information has been compromised? Follow these 3 steps to report it.
Step 1:
Click the button above to find the federal government’s one-stop resource to report identity theft and get a recovery plan or browse recovery steps. Your online identity theft report will help create a personal recovery plan.
Step 2:
Visit www.identitytheft.gov to fill out the Identity Theft Victim’s Complaint and Affidavit. Click the button below for a helpful resource guide. NOTE: The link below does not contain a fillable online form.
- Email a copy of your completed form, documents, and the law enforcement report to the Missouri Attorney General’s Office. Email: consumer.help@ago.mo.gov
- Provide copies of this form, documents, and the law enforcement report to credit bureaus, and attach letters disputing fraudulent charges or acts.
- Keep a log of all actions taken
If you have completed the steps on the www.identitytheft.gov website And completed the Identity Theft Victim’s Complaint and Affidavit, you can make your online Identity Theft Report.
Step 3:
Use the Nixa Police Department Online Police Reporting (NPORS) to complete an online identity theft report and attach copies of this completed form and documents that you can choose to review, print or email for your records.
Note: If you have already filed a report with an officer, you must return the completed 6-page Identity Theft Victim’s Complaint and Affidavit (starting on Page 16) either online, in person, by mail, or email. All correspondence must include your case number and contact information. Any cases that cannot be verified or the affidavit is not returned will remain cleared and kept on file.
